Qualifications
~ Education: Masters in Nursing (FNP|AGNP specialization)
~ Licensure: Active NYS NP license (or eligibility to obtain)
~ Certification: Board Certified|Eligible (FNP, AGNP, or equivalent)
~ Experience:
~ Preferred: Clinical experience in adult|geriatric care
~ Experience managing chronic diseases (e.g., diabetes, dementia)
~ Technical Skills:
~ Patient assessment, diagnosis, care planning
~ Medication management and EHR documentation
~ Soft Skills:
~ Strong interdisciplinary collaboration
~ Effective patient|family education
~ Attention to detail in clinical documentation
Key Responsibilities
~ Conduct comprehensive assessments and develop individualized treatment plans
~ Prescribe and monitor medications, addressing dosages and potential side effects
~ Manage chronic illnesses such as diabetes, hypertension, and dementia
~ Provide acute care for infections, injuries, or exacerbated chronic conditions
~ Collaborate with physicians, nurses, social workers, and families to coordinate care
~ Communicate clearly with families regarding patient conditions and care plans
~ Educate residents and families on treatment, prevention, and wellness strategies
~ Maintain detailed documentation in compliance with legal and quality standards
~ Participate in facility quality improvement initiatives
